April 20 2012 - June 3, 2012

4-20-12 Friday. Foxy & I arrived at NH about 9:20 AM. Mom was sitting in w/chair in her room. I got her walker and put her feet up. I gave her a cookie & put Foxy on her bed so she could reach him to pet him. C N A came to get her for a bath. I saved half her cookie while she went. I put Tom Hanks Cast Away movie which Riley had cleaned on DVD. C N A took conditioner that Helen had bought. It worked nicely on mom’s hair.  We put on hand cream. Some of Mom’s toiletries are missing (deoderant). C N A put Mom in bed. We did some exercises; legs and cloud hands. We left with dirty clothes about 10:30 AM.

4-21-12 Saturday. Foxy & I arrived at NH about 3 PM with the clean clothes. I cleaned her glasses, brushed her hair, soaked and cleaned fingernails. We got Mom out of bed with assistance from C N A, who also changed Mom’s diaper in bed. We accompanied Mom to Dining room II for piano recital by youngsters. Mom propelled herself most of the way. Recreation therapist assisted in return to Mom’s room.  Together we hung up clothes and put in her closet. Then we accompanied her to dining room for supper. We left about 5:50 PM.

4-22-12 Sunday. Foxy & I went to NH about 10:30 AM. Mom was dressed, in bed. We got her up, changed TV from DVD to TCM. I put cream on her, cleaned glasses, wiped eyes & face and brushed her hair. Nurse came and gave her a dose of an inhaler, apologizing for being late. We took Mom out to the covered patio where we listened to the birds then Foxy & I accompanied her to dining room for lunch. We left about 11:50 AM. Foxy and I returned about 6:40 PM and Mom was in bed watching TCM. I changed TV to DVD, brushed Mom’s hair, cleaned glasses, adjusted bed and put Foxy on her bed. I gave Mom a cookie and Mom gave Foxy a treat, petted him. Carl and Charlotte came with new flowers. We had a good visit. More adjusting of Mom’s position in bed. We left about 8:20 PM.

4-23-12 Monday. Foxy & I arrived about 11:00 AM. I changed the TV to channel 8, local news. Mom had seen the dentist and was awaiting transport back to her room. Foxy & I took her back to her room briefly then accompanied her to the lunch room. We played Pease porridge hot. Foxy and I went to see Mom about 7:30 PM. I changed TV to DVD from TCM.  I adjusted Mom on bed, cream on her feet and massaged. A few leg lifts and toe wiggling then I put socks on her. She was not interested in Foxy tonight. We sang a couple of songs then Foxy & I left about 8:10 PM.

4-24-12 Tuesday. I arrived  

4-25-12 Wed. Foxy & I visited in the morning about 11 -11:50 AM when we left Mom in dining room. I cleaned her glasses and changed TV to news. Then I returned about 8:00 PM and Carl and Charlotte were there too. I left about 8:30 PM.

4-26-12 Thursday. I arrived about 11:00 PM. Mike showed me two rooms to choose from. I chose 129 for Mom. I left about 11:50 AM. I returned about 5:30 PM. and accompanied Mom to supper and assisted her eating. (Pizza).

4-27-12 Friday. I went to NH this AM about 10:50 AM. I found Mom in bed watching DVD. Bed rails on both sides of her bed were down. I left them down but asked the nurse as the bed rails previously had always been up. Nurse explained that this is a no restraint facility and bed rails are a form of restraint. I said Mom has fallen 4 times. I changed channel to KLRU cartoons (Mom said she wanted to see cartoons). I returned to NH about 7:10 PM. Bed rails were up. I cleaned her glasses, picked up laundry to be washed. TV was on TCM. I left on TCM.

4-28-12 Sat. Foxy & I went to NH about 9:50 AM. Mom was in bed watching TCM. No teeth. I got her teeth from Efferdent bath in plastic cup in bathroom, brushed them and gave to Mom.  Her glasses not on her and were especially dirty. I cleaned them, brushed her hair, adjusted her bed. We scrolled through TV channels then went back to TCM. Nurse gave Mom inhaler treatment. Mom has difficult time understanding (Caribbean accent).  I went to check out Mom’s room to be, Rm 229 but did not go in. We left about 10:30 AM. I returned about 4PM with Mom’s laundry. We hung it up and changed the channel on TV from TCM to DVD. I brushed mom’s hair, cleaned her glasses and called the C N A, Precious, to help Mom to bathroom and change her dress (wet on hem). I visited Room 229 and met roommate to be who was being hand fed in bed. I measured space for TV. It is a fit. Then I took Mom around the NH and we sat for awhile on the covered Patio. I left her in the dining room about 5:15 PM.

4-29-12 Sun. Foxy & I went this AM about 9:30.  Mom was dressed, in bed, no teeth with a partially eaten peanut butter-jelly sandwich on bedside table. I got her teeth (soaking in Efferdent) brushed them, gave them to her and gave her the sandwich. She ate that. I cleaned her glasses. Mom gave Foxy a treat. I left Foxy on bed with her while I tried to find the other room but I did not identify it.  We left about 10:20 AM. I returned about 6:20 PM.  Mom was at a dining room table with another resident, her meal mostly not eaten. No oxygen. Mom was being argumentative, refused to taste food and did not want to return to her room, either. Nurse reported to me that Mom had been propelling herself all over the place. Mom had leaves in her hair and in her room the plastic tubing was attached to portable O2, indicating someone had accompanied her somewhere. I got her music and we went to piano for a bit. Then we returned to her room with her propelling her wheelchair with hands.  I got O2 on her with argument.  She colored for awhile then with chocolate she agreed to go to bathroom with Juanita, CNA, and I assisting and changed clothes. I cleaned her glasses, brushed her hair, changed channel to Fred Astaire DVD and left about 7:40 PM. She also did some leg lifts.

4-30-12 Monday. Foxy & I arrived about 9 AM. We saw Tammy & Mike about upcoming room change. I looked at 3 rooms and met the roommates. Then we went to see Mom. Mom was asleep in her bed with damp hair, DVD on TV. Mom was very tired after her “manic” day yesterday. CNA and Nurse reported Mom was very verbal, conversational yesterday. All seemed pleased by interactions. I cleaned her glasses, made her sit up while I put conditioner and brushed her hair. Mom gave Foxy a treat and petted him furiously. I cleaned her eyelids and glasses but Mom couldn’t keep her eyes open. I adjusted her bed, left TV on DVD and left about 10:20 AM.

5-1-12 Tuesday. I went to NH after work arriving about 12:05 PM and told Administration that I had decided on the room in the back, Room 206, I believe. Mom was at lunch without her wheelchair. She had a belt around her waist and her walker nearby. Therapist told me that Mom had walked, with rest stops, to the lunch room.  I assisted Mom with salt & pepper. Mom was eating well.  I left before she finished eating about 12:40 PM. Foxy & I returned at 3:00 PM and accompanied Mom to Crafts at 3:30 PM. We accompanied her back to her room about 4:30 PM and we left shortly thereafter.

5-2-12 Wed. Foxy & I went to NH about 9:30 AM. Mom was in bed with hair damp so I put conditioner on her hair. Cleaned her glasses and gave them to her. I left TV on.  I went again in the afternoon about 5:30 PM. Mom was in dining room. I accompanied her to her room. Mom ate well.  I took out her teeth, brushed them. I put TV on TCM. I left about 6:30 PM.

5-3-12 Thursday. Foxy and I went about 4:15 PM. I paid bill to administration. Mike said we could fix up her room on Sunday. Mom was in hallway with Med nurse near the dining room Without glasses Nor teeth Nor oxygen. We accompanied her back to her room. Mom propelled herself. The oxygen was not in her room. Mom gave Foxy treats, I changed TV from TCM to DVD on mute.  I got her teeth which was in the Listerine water mix from night before. Her glasses cleanser is gone. Also her bib was not to be found. So I put dishtowel on her and Foxy & I accompanied her to dining room. Her oxygen was in the dining room.  Nurse reported to me that Mom had been very active today, played piano and propelled herself around the unit. Mom did not eat well. We returned to her room and ate cookies, played peas porridge hot. CNA Angela came and helped put Mom to bed. Mom’s diaper was dry. I turned up sound on DVD, Mom petted Foxy. I gave Mom a hard candy and we left about 6:30 PM.

5-4-12 Friday. I went to NH about 10:40 AM. Mom was in bed with bed rails down. Nurse reported that Mom had been getting out of bed and bed rails were dangerous, further to fall. I put tea tree oil on her face growth, cleaned her glasses and talc on her. I changed the TV from DVD to TV on mute. We started to color when Riley arrived. We chatted and I left about 11:20. Riley reports that Mom drew portrait of him. I returned about 6:45 PM. I left about 7:40 PM. I left TV on DVD.

5-5-12 Saturday. I went to NH about 10:00 AM. I cleaned her glasses, washed her teeth, brushed her hair. I found her glasses cleaner in her sock drawer. CNA helped me get Mom up. Her dress was wet so we changed her dress, too. Mom had on her bra which I had thought was gone! We went on a trip around the NH.  I left her O2 in dining room. We went to Rm 205 and met her new roommate, Tina, and the charge nurse on duty. I left Mom in Dining room about 11:40 AM. Other resident pointed out Mom needed a sweater. I looked in her room but no sweater so I got her a shirt.  Foxy & I returned about 3:20 PM. I brought two sweaters and she put one on. We accompanied Mom to the music program in Dining room 11. Tina was there at a table with 2 other ladies. We chatted. After program Mom & I stopped at piano in Dining room 1 and Mom picked out some music. We left Mom in dining room with O2 about 5:20 PM. I picked up dirty clothes to wash.

5-6-12 Sunday. Today scheduled to fix up new room. I went this morning about 10:40 AM and put Mom’s collage and some photos on the wall plus her clean laundry in her new room to be.  Still no bed for Mom in the room. I went to see Mom. The CNA was in her room getting Mom up and into the wheelchair. I cleaned her glasses, got her teeth, brushed her hair and asked CNA to leave her with me, that I would take her to lunch later. I changed from DVD to TV and channel 8 news. I left about 11:55 when her lunch arrived. Riley & I returned about 3:15 PM with Mom’s TV. Tina was not in her room. I saw her at Memorial service. We hooked it up in her room. The cable is a little short but it works. We went to see Mom. She was sleeping. She got up readily to go to see a program with Riley and I. I cleaned her glasses and we went to the Episcopal service in dining room 1. Riley left and I stayed for the service.  Barbara W. was there playing piano for the hymns. I accompanied Mom to her room. I changed the TV to DVD. Carl & Charlotte came with chicken nuggets and treats. I left about 5:20 PM. Carl & Charlotte were still there.

5-7-12 Mon.  Foxy & I went to NH about 9:15 AM. I spoke with Mike the administrator about no bed in new room. He said he would see to it. I also mentioned to Tammi about prospects of borrowing O2 machine for a visit. I also have a call into Dr. Maxwell about Mom’s diagnosis, prognosis. Mom was in bed watching DVD. Mom’s hair was slightly damp. I put on conditioner, cleaned her glasses. We got her up and took her to Texercise/trivia in Dining room II. Mom kept falling asleep as she didn’t follow verbal jokes and instruction. We left about 10:30 AM. I was pulling O2 with Mom behind me. She lost control on the slope leaving the dining room and ran into Nurses station with wheelchair. I had put “drags” on wheels but not enough drag for that slope. I took her back to RM 120 and put her back in bed with DVD on TV. I left about 10:50 AM. I talked with Dr. Maxwell this afternoon by phone. Doctor said prognosis for Mom a year or so and d/cd O2 all the time. She said fingernail polish was skewing the O2 readings. I returned about 6:10 PM. Mom was in dining room with one other resident. I coaxed her to eat some more, helping to put pepper on her food. Then accompanied her to her room. Her bed stunk so I got a new pad and bottom sheet and put on her bed. Tonight I couldn’t find the glasses cleanser again. I left case with tea tree oil and new glasses cleanser in bottom drawer. I gave Mom a chocolate and she gave Foxy a treat. CNA Angela helped put Mom in bed, changed her. We did some kicks and I put Oxygen on her for sleeping only now. Now Mom can go out for short visits. After leaving Mom I went to room 205 and there is a bed in the room, now.

5-8-12 Tues. I went to NH about 11:30 and told Mike the Administrator that we will move Mom today. I went to Mom’s new wing and introduced myself to charge nurse and CNAs. Then went back through dining rooms and Mom was in dining room. I gave her 2 cherry tomatoes and basil leaf then lunch came. Mom ate okay. I started on her room. Riley came and got small chest of drawers on Johnson bar (dolly). Mom & I carried stuff to her to new room. Riley worked on TV/DVD set-up and furniture moving. New bed is not electric but a crank bed. Tina is a fountain of information on people and things. Riley left about 2 and I left about 2:50 PM. I went to back and put birdseed in feeder. Foxy & I returned about 7:10 PM. Mom was in bed watching DVD on top of covers. Evidence that Mom ate in her room. I pushed button for assistance and CNA, “Angel”, came and changed Mom in bed and put her in bed. She put bed rail up on one side. We discussed bedrails and falls. I brushed her teeth and put them by her bed and changed DVD to Fred Astaire/Ginger Rogers. Foxy & I left about 7:50 PM.

5-9-12 Wed.  Foxy & I went to NH about11:30 AM. CNA, Eric, had Mom up and dressed in turquoise outfit but no teeth. We couldn’t find her teeth. They were under the stuffed dog. I accompanied Mom to lunch room. I helped her eat then we left after food was served about 12:30 PM. I returned about 5:45 PM. I put birdseed in birdfeeder.  Mom did not want to get up but I got her up and accompanied her to lunchroom for supper. She ate well.  I left Mom in the sitting room about 6:30 PM. She said she was cold so I gave her pink sweatshirt/hoodie and a blanket for her lap. I left by door in sitting room.

5-10-12 Thursday. Foxy & I went to NH about 3:20 PM. Mom was in bed with O2 and pink sweatshirt/hoodie. Mom did not want to go to activities. She wanted to stay in bed in watch “this good show”. She was very contrary. It was some kind of “reality” cop/ghosts show on TLC. Foxy & I left about 3:30 PM.

5-11-12 Friday. Foxy & I went to NH about 9:30 AM Mom was in bed watching TV. We put on hand cream, cleaned her nails, glasses. We left about 10:30 AM.

5-12-12 Saturday. Foxy & I went to NH about 11 AM. Mom was in the sitting room. We went outside but it was cool for her so we went back inside. I didn’t find a sweater so I put her towel on her lap. I got her colors from her room and she colored at the writing desk. Then we took her into lunch room and I got her a coffee. Foxy & I left about 11:50 AM. I returned about 3:15 PM. Mom was in bed watching DVD Shall we dance. I got her up (chocolate) and took her to Dining room II for entertainment. I got her some juice. We sang and clapped to the Texas Stars. I took her back to the room and put her back in bed. I left about 4:40 PM.

5-13-12 Sunday. I checked Mom out of NH about 11 AM and returned to NH about 3:30 PM. She was dressed and ready to come home for visit.

5-14-12 Monday. Foxy & I went to NH about 11 AM. Mom was in hallway without her teeth. We went back to her room, searched and I found them in a Styrofoam cup. DVD player was on but TV was on something else. I cleaned her teeth and she gave Foxy a treat. I opened the blinds. Mom said she was cold so I found a sweater for her. I went to the car and got birdseed for the feeder. Mom watched as I filled the feeder and the birds came. We watched a sparrow feeding juveniles. We accompanied Mom to the lunch room. Foxy & I left about noon. I returned about 6:30. Mom was in her wheelchair in her room, no TV. I found something on TV and got Mom colors and coloring book. We put her feet up and I put cream on her feet, cleaned her glasses. We discovered the call button did not work but the call button in the bathroom worked. CNA, Juliana, helped Mom to bed, changed her in bed. We discovered there was no tubing on the oxygen machine. I told the nurse, Willie, and he brought a new one.  I cleaned her teeth and left them in water by her bed. I left about 7:40 PM.

5/16/12 from Carl -Hung up 7 dresses for mom about 2:00 pm.Riley is the one who told Tina about 4&4.from the pictures on the wall.he's feeding the birds too.Char's staying in bed today so we won't go to Pappy's tonight to see Riley play.

5/30/12 Wed. Foxy & I went about 5:45, filled the birdfeeder. Mom was at supper. Nurse Steve let Foxy stay at Nurses’ station while I sat with Mom at supper. Mom didn’t eat well. She is losing weight. We went to film night. Carl & Char came and we went to Mom’s room. I left Fred Astaire on DVD.

5/31/12 Thur. I went to NH about 11:30 AM and took chicken salad to Mom for lunch. We had lunch in her room. She ate all her portion plus half a banana from Carl. We went for a walk. Mom propelled herself to covered patio. Birdseed in feeder attracted a bluejay, sparrows, rock dove and squirrels. CNA assisted putting Mom to nap.  I put Singing in the Rain on DVD. I left through the front about 1:40 PM.

6/1/12 Fri. Foxy & I went to NH. I got Mom up and into wheelchair. Dentist came to visit Tina and he gave me instruction on using Fixodent properly. We went to the covered patio, Mom propelled herself. I found one of her dresses on the laundry rack.

6/2/12 Sat. I went to NH about 10 AM. I put birdseed in feeder then went inside. Tina was gone Mom was watching Singing in the Rain on DVD. Mom did not want to get up. I changed it to TCM when I left about 11:10 AM. I returned about 5:20 PM. Mom was in the dining room. Tina and Donna reported to me that Mom attended music program this afternoon. I couldn’t find her glasses then found them on the window ledge. Nurse discovered no labels for Mom’s possessions in book although they had been ordered. I assisted Mom with eating. She ate lightly but benefited from salt, pepper and a cup of coffee. I accompanied her to her room. Vanessa, CNA, came and put her to bed, changed her. I left DVD on TV.

6/3/12 Sunday. I went this morning and put birdseed. Mom didn’t want to do anything. TV was off when I got there. I put on Cast Away and left. I returned about 4 PM and Mom and I drew each other in dining room. Her walker is missing. Charlotte & Carl came and brought flowers. I got peanut butter sandwiches for Mom for supper.

6/4/12 Mon. I went about 9:30 and put out birdseed. I found her walker in my car’s trunk.

6/5/12 Tuesday. Foxy & I went in the morning.  Mom was weighed and she has lost about 35 lbs. (150). I went to NH at supper and assisted in eating but Mom mostly refused. Call light was not working. Diane, Nurse, advocated for half portions for Mom at my behest.

6/6/12 Wed. I took fruit salad to NH about 10:15 AM. Added birdseed to feeder. CNA had shaved Mom and she looked great. We went to dining room and Mom ate well in dining room without distractions. I advocated for giving Mom her meals in her room with reduced distractions. I returned about 5:45 PM to find Mom eating in her room. She ate well. Nurse Steve reported that she ate about half her lunch in her room. That’s an improvement.

6/7/12 Th. Foxy & I went to NH about 9:30 AM. We got Mom up and took her to covered patio and played pease-porridge-hot. Then we accompanied Mom to activity in big room (chair dance). I spoke to Donna and will seek her out tomorrow about 11 AM about volunteering. Riley called 9 PM to say that he had seen Mom, took out her teeth.

6/8/12 Friday. Foxy & I went to NH about 10 AM. I put birdseed in birdfeeder. Mom was in bed, no TV. I got her up and we went to Nail Spa then Bell Ringers. Mom participated in bell ringers. I made agreement for volunteering with Donna. We left about 11:30 AM I left Mom sitting up in her room coloring and watching TCM.

6/9/12 Sat. Foxy & I went to NH about 11 AM. We put birdseed in feeders. Mom was in bed with TV off. I got Mom up. Tina reported that Mom did not eat well at breakfast. If I understood correctly, she was taken to dining room for breakfast. We looked at weather channel then I changed to TVLand. CNA Maria came to toilet Mom. I assisted to bathroom. Then she brought Mom’s lunch. I turned off TV and assisted Mom with lunch. Then Foxy and I accompanied Mom to covered patio. Bight sunlight bothered Mom but she enjoyed the enclosed forest (shady). Mom propelled herself. We returned to her room.  I put her to bed. I brought clothes home to wash. We left about 1:15 PM.

6/10/12 Sunday. Foxy & I went to the NH about 9:30 AM. I turned the birdfeeders around, over grass instead of sidewalk. I hung up her clean clothes. Beautiful flowers from Carl on Tina’s dresser. Tina is thrilled. We checked out Mom for Lunch. We went by Sharon’s then the store then home. Lu called. We sat on back porch then back to NH about 1:30 PM. Mom leaked. I left TV on DVD and Mom in bed.

6/11/12 Mon. Foxy & I went in the morning. Mom still in bed with breakfast. I got her up and took to a sing-a-long. Left her there.

6/12/12 Tuesday. Foxy & I went about 2:15 PM. We accompanied Mom to B’day party then we left. Clothes on Patio.

6/13/12 Wed. Foxy and I went to NH about 10 AM and accompanied Mom to coffee hour with entertainment by Religious songs, guitar. Mom was in bed. I cut her toenails. We left Mom at coffee hour.  Residents were waiting for toenail clipping.

6/14/12 Thurs. Foxy & I went to NH about 9:30 AM. Mom asleep with TV on. I put birdseed in feeder and got Mom up. Riley arrived with a strawberry milkshake which Mom loved! I sketched Tina while Mom worked on shake. Then we accompanied Mom to recital of recorder consort. Wonderful music. Foxy & I left about 10:45 AM.

6/15/12 Fri. Foxy & I went to NH about 9:30 AM. Mom sleeping. Nurse Elvia reported that Mom was not eating again. We discussed problem. We accompanied her to program for making butter plus she had rehearsal for bell ringing. I returned about 2:50 PM. I brought molasses, Tabasco steak sauce, Pepper, curry and left at nurse’s station to flavor her food. I transported Mom to program, Jon Emory performance. I did 5 sketches of residents (Ran, Pat, Mary E., Eleanor, and ?)  during programming. Cheese & crackers served in honor of Cheese Month. I accompanied Mom back to her room and left her sitting up watching DVD.

6/16/12 Sat. Foxy & I went to NH about 2 PM. Nurse Steve reported that Mom ate well at breakfast (molasses). I filled birdfeeder, turned up sound on DVD and Mom cracked pecans until 3:15. I transported her to program of dance performance by 3-5 years olds. I took clothes home to wash.

6/17/12 Sun. Foxy & I went to nursing home this morning about 10 AM with clean clothes. Filled birdfeeder. TV on TVland. I put on DVD, Swingtime. Foxy scratched Mom on leg, broke the skin. I told nurse that I did it. Nurse reports that Mom not eating well. I returned to NH about 2:50 PM and got Mom up and ready for party. I escorted Mom to Dining room II and left her in line with Bell Ringers. Carl, Charlotte and Kim came to performance & party. Cookies and icecream floats were served. I did sketches of 4 residents. I escorted Mom back to her room and left her watching TV in her wheelchair about 5 PM. Carl added feed to birdfeeder. About 8 PM Nurse Steve telephoned to say that Mom cut her leg trying to get out of bed. I suggested to put DVD on TV.

6/18/12 Mon. I went to NH about 10:15 AM. Mom leg was leaking. She was in bed in jamas, no teeth, sleeping and watching DVD, no sound. Nurse Luz changed her bandage. I got assistance from CNA Maria and another and got Mom up into wheelchair. We went for a walk and Mom said that she wanted to eat in dining room! I left Mom in dining room about 11:30 AM. I fed the birds.

6/19/12 Tuesday. I went to NH about 9:30 AM. Mom was in bed with TV on TVland. I changed to DVD, Singing in the Rain. I fed the birds. Mom was dressed with teeth. I poked her leg and asked about pain. Answers were contradictory.  I accompanied her to activity. She played ball and I sketched residents. Mr. Peterson’s daughter gave me a $5 tip which I gave to Ann for Rec. petty cash. I left Mom at Nursing station about 11:30 AM. I returned to NH about 8:20 PM, fed the birds. Mom was in bed without teeth, TV on TV land. I changed to DVD.

6/20/Wed. Foxy & I went to NH about 9:30 AM. I fed the birds. I accompanied Mom to entertainment. I left her at table with Tina. I returned about 7 PM. I fed the birds. Mom was sitting up in her wheelchair watching DVD. I called for help and Juana helped me put Mom in bed and changed her. I got Nurse Steve to look at Mom’s bandage because it slid down her leg loosely. He did not have access to needed treatment so he taped the bandage in place and said that it would be changed Thursday.

6/21/12 Thurs. Foxy & I went to NH about 9:30 AM. I fed the birds. Mom’s bandage had not been changed. New flowers in the vase. Mom was in bed. CNA Juana & I got her up and Foxy & I accompanied her to Stretchies group in big dining room.  We left about 10:30 AM. I returned about 7 PM. Mom was sitting up in wheelchair watching DVD. Vesta came and we accompanied Mom to bathroom for BM.  We put her to bed. Bandage had not been changed. I got Nurse Steve to come and change her bandage. I left about 8:20 PM.

6/22/12 Friday. I fed the birds. Mom was in bed when I got to NH about 9:45 AM. Bed was very wet. New C.N.A. was there starting to get Mom up. I assisted to get her up. I found her upper teeth on floor. I brushed her teeth. Nurse Elvita came and changed her bandage. I accompanied Mom to exercise program then took her back to her room. I left Mom in her room, in her wheelchair watching TCM. Another resident who Tina reports as being predatory was hanging around the room entrance. Tina reports that this woman has entered room during the night and slapped her (Tina) once. The woman’s name is ? Thin, self-propels.

6/23/12 Sat. I fed the birds. Foxy & I went to the NH about 9:30 AM. Mom was in a good mood. We cracked nuts. The C.N.A. helped get Mom cleaned up and dressed. We accompanied her to Games & Craft program. I got Mom’s colors and left her in big dining room coloring. Jan thanked me for helping the previous day at the exercise program. I left TV off but on AUX channel.

6/24/12 Sunday. Foxy & I went to NH about 9:30 AM.  TVLand on TV and Mom in bed but alert. I fed the birds. I changed TV to TCM. We cracked nuts.  I accompanied Mom to big dining room where we listened to Ellen (resident) practice the piano. I accompanied Mom back to her room and left her with colors watching DVD. About 11:30 AM.  Bandage in good shape.

6/25/12, Mon. Foxy & I went to NH about 9:10 AM. I fed the birds. Mom was in wheelchair with TV off. I turned it on and DVD Pirate was on so I changed it to Fred Astaire, cleaned glasses, brushed Mom’s hair, etc. and accompanied her to Sing-a-long activitiy. I got her a coffee, talked to nurse about Mom’s condiments. Foxy & I left 10:30 AM.

6/26/12 Tuesday. Foxy & I went to NH this morning about 9:10 AM. Mom was sitting in wheelchair watching DVD, no sound, no teeth. We looked everywhere and could not find them. Nurse Diana looked also and noted all the contraband in the room. Administrator Mike told me about Carl’s transgression against Tina (looking in her drawer and recording her phone number.) Foxy & I left about 11:10 AM.Sheila C.N.A. on duty said that she was unaware that Mom had dentures and had not seen them. Tina reported that she had heard something about 5:10 AM; that the noise could have been something dropping.  I returned to NH about 3 PM. Nurse Diana told me to remove ‘contraband’ like mouthwash. Mom was in bed watching DVD. I accompanied Mom to activity and I sketched portraits of 3 residents. We returned to her room. Dr. Fran told me that she felt that Mom’s weight loss was due to loss of fluids. Riley came to visit. He left and said he would return with shake from Sonic. I tried playing new DVDs from Brooks but they would not play. Supper was brought early. Mom had difficulty masticating without teeth.

6/27/12 Wed. No teeth. Morning visit. Sheila C.N.A.’s comment “I didn’t know she had false teeth” .

6/28/12 Thursday. Foxy & I did a Morning visit.

6/29/12 Friday. I arrived at NH 8:50 AM. I fed the birds. Mom was in the TV room with other residents. We went back to her room and did her nails. Mom said she needed to go to the bathroom (BM). I called for help but Mom & I did the toileting without assistance.  Singing in the Rain DVD is failing. I put on Flying to Rio. Med nurse brought supplements and meds, took her blood pressure. I gave Strawberry flavored syrup to Nurse.  I accompanied Mom to Fit & Fun program then talked with the nurse Diana about dentures for Mom. Dr. Jackson and Dr. Khan do dental work for RNC. I left about 10:40 AM. Nurse Diane said I could leave “precautions” (not “contraband”) in locked case in top of Mom’s closet. She also said that Mom’s albumin level is 3.1, up from 3.0 when she was first admitted. ONLY a gain of 0.1!

6/30/12 Sat. Foxy & I went to NH about 9 AM. I fed the birds. Mom was sitting up in chair, not dressed watching TCM news. I got her a dress, brushed her hair. I put locked overnight case in top of closet. I searched for clean pillow case for her big pillow and could not find it. We left about 9:45. I returned about 6PM. and put on DVD of Winged Migration. It doesn’t work. Two CNAs came and assisted putting Mom to bed. I put on DVD of Fred Astaire. I left about 7:30 PM.

July 1, 2012, Sunday. Foxy & I went to NH about 9:15 AM. I fed the birds. I looked in on Mary E. but she wasn’t home.  Mom was in bed, dressed. TV off. I assisted Mom to bathroom where she had small BM.  I put TV on TCM. Musicals today. We cracked nuts. C.N.A. took Mom to weigh and reported that Mom was 146 lbs. (or 148?). We left about 10:40 AM.

July 2, 2012, Monday. Foxy & I went to NH about 9:30 AM. I fed the birds. Mom was dressed and in her chair. I cleaned her nails, face. Med nurse brought Boost and took her blood pressure. I got strawberry flavoring from Nurses station for the drink then returned it. We accompanied Mom to big dining room and I got her some coffee. We played pattycake and ‘here’s the church’. I left TV on weather channel. We left about 10:15 AM as they were setting up for Texercise. Foxy & I returned to NH about 6:30 PM. I fed the birds. Mom was sitting in her w/c in her room, TV on, sound down. We did some exercise. I tried new DVDs but they didn’t work. Mom colored very nicely today. Sheila C.N.A. came to put Mom to bed. She removed Mom’s dress abruptly without pulling blinds or closing curtain. This C.N.A. was singularly careless and unprepared. No pad for bed. No towel. Wheelchair askew and positioned far from bed. We got Mom in bed and positioned poorly. Tried again. Less than ideal (Mom to one side.) “Your mother jus talks to me when you’re not here”. ??? That is odd for Mom? I put on another DVD (Shall we dance). Foxy & I left with clothes to wash about 7:30 PM.

July 3, 2012, Tues. I found missing pillowcase in dirty laundry. Foxy & I went to NH about 11:40 AM. I fed the birds. Mom was in her room with pureed Lunch and TV on. I turned off TV and got her spices from Nurses station (Elvita). I added about ½ teaspoon of curry to macaroni & cheese and ½ teaspoon Steak sauce to pork dish. Mom ate about ¾ of those two dishes. I left TV on DVD. We left about 1 PM. Charge Nurse Diane left message on my machine to call Lauren, speech Therapist. I called and talked to Speech Therapist, Lauren, about eating issues. Lauren says that she is new to this facility. I went to NH about 7:40 PM. I fed the birds. Riley has made some repair to feeder. Mom said she was thirsty so I put table with water near her, within reach. Mom and I put her clean clothes on hangers. I changed the DVD and opened the blinds. I left about 8:10 PM and set off alarm on the TV room door.
